Below is the Accident/Incident Form for the University.
This should be downloaded, completed, and e-mailed to the Health & Safety Office (hands@londonmet.ac.uk ). The Health & Safety Office will forward copies to the Insurance Manager in Finance and to HR as appropriate.
Accident/Incident Reporting Procedure 2011
Certain accidents and incidents must be reported to the Health & Safety Executive.
If necessary, this will be done by the University Health & Safety Office via the RIDDOR Incident Contact Centre.
